Telemedicine or Virtual Visits
Due to Coronavirus (COVID-19) situation, we are offering virtual appointments using a HIPAA compliant Telemedicine platform.
- Before your appointment, please login to your Patient Portal and sign the Telemedicine Information and Consent Form and Credit Card Authorization Form.
- Before your appointment, please call us to put a 'Credit Card on File' for copay, coinsurance and other charges per our policies. We will email you a receipt when we run your card.
- A few minutes before your appointments, click on the link for your provider to start a virtual visit with your provider. No software install is required. It works best on Chrome and Firefox browser on your laptop or mobile device.
- Please note that your appointment time is the time blocked on your provider's calendar. All other tasks (card on file, starting tele session) must be completed before that. This allows us to reduce wait time for all patients to serve them better.
- Please know that we will do our best to maintain a private location during your session. It is your responsibility to ensure privacy in your location as well.
